Search for Bernard on the 4x4 Community Forum, he borrowed mine a scanned it. He was trying to post it on this forum but did not know how to or who to contact.

I got mine from Mitsu Brooklyn. You will have to order it as they don't keep it in stock. Can't remember the cost, however.
